The agreement you sign means that it will be accessible as part of the Open Access database of dissertations and anyone in the world can read your dissertation via Google Scholar or other open access search engines. Yes - you should include it on your vita and list it the same way you would if you were citing a dissertation in APA style in a bibliography. If you have published any articles or if you have any UNpublished works you should also list them, but note when citing them that they are "unpublished".
